For all New Clients, please complete the New Client Form and bring it in with you on your first appointment. Copies are available in the office, so you may complete one there as well if are unable to print it out beforehand.

Oncology Support
In addition to the New Client Form, please complete the Additional Client Info Form as well. Due to the thorough nature of this form, completing it ahead of time is highly recommended.

Please note that if you are currently in treatment, or if your last treatment session was less than 12 months ago, it is important that your physician completes the last page of this form prior to your first appointment. Thank you.
